Use this form to submit a verifiable privacy request under applicable state privacy laws, including the California Consumer Privacy Act as amended by the CPRA.

Before you submit

Kizuna provides workflow software used by employers to review background screening information they receive from screening providers. Kizuna is not a consumer reporting agency, does not create, modify, or furnish background reports, and does not make employment or eligibility decisions.

If your request concerns the accuracy or correction of a background report, contact the background check provider named in the report or notice, or the employer directly — not Kizuna.

Where Kizuna processes candidate-related information through the platform, it generally does so on the employer's behalf as a service provider / processor, and we may direct you to the employer or originating provider as appropriate.

Identity verification

To protect your privacy, we may need to verify your identity before processing your request. We may contact you at the email address provided to confirm your identity and the details of your request. We will not ask for your Social Security number, financial account numbers, or login credentials as part of this verification.

By submitting this form, you represent that the information provided is accurate and that you are authorized to make this request. Kizuna will respond to verifiable requests within 45 days, as required by applicable law. We may extend this period by an additional 45 days where reasonably necessary, with notice.
